Decoding the role of private providers in hypertension care in rural and peri-urban India

Hypertension has been the single largest cause of death in India over the past 15 years. At the same time, private providers are the first point of contact for curative healthcare for over 70% Indians. It thus becomes imperative to understand the private providers' landscape in hypertension care.
MIHM, with Resolve to Save Lives, is conducting an exploratory qualitative study in rural and peri-urban Telangana to understand the current practices and incentive systems. Respondent groups include modern medicine providers, AYUSH practitioners, rural medical practitioners (RMPs), labs, pharmacies, and hypertensive patients. The results indicate that RMPs – the first point of contact for most people – lack technical knowledge as well as the legal status to prescribe medication. Across providers, there is a variation in criteria for categorising patients as hypertensive and the structure for monitoring and follow-ups is missing. On the patient side, lack of awareness and high out-of-pocket expenditure are key barriers to screening, diagnosis, and adherence to follow-ups.
The study is currently in Phase II where we are trying to outline interventions for better engagement of these private providers.
Value-based human-centered systems for diabetes care in India

India has the second-highest number of diabetes cases globally, with the number projected to touch 100 millio by 2030.
MIHM along with Leapfrog to Value conducted a landscape study to understand the barriers and health system challenges in the delivery of diabetes care. Conducted using secondary literature and primary interviews with various stakeholders such as patient advocates, doctors, insurance companies, and startups, it showed the barriers on the patient side include low awareness, long commute and wait times. On the supply side, there is a dearth of diabetes health professionals, and the providers are not united on the guidelines to follow. Diabetes self-management is tedious and suffers from low compliance and high OOP expenses as insurance does not cover OPD/continuum of care.
In December 2021, we conducted a stakeholders' roundtable where two global interventions were discussed that could inspire models in India. As a follow-up, we aim to undertake a pilot intervention by partnering with relevant stakeholders that would incorporate principles of value-based and human-centered care to better meet the needs and interests of patients, payers, and providers. A position paper highlighting the insights is underway.
