Can India battle hypertension by task-sharing models with private pharmacies?
Low density of formal care providers in rural India results in restricted and delayed access to standardised management of hypertension. Task-sharing with pharmacies, typically the first point of contact for rural populations, can bridge the gap in access to formal care and improve health outcomes. ISB MIHM implemented a hypertension care programme involving task-sharing with 20 private pharmacies between November 2020 and April 2021 in two blocks of Bihar. Pharmacists conducted free hypertension screening, and a trained physician offered free consultations at the pharmacy. We calculated the number of subjects screened, initiated on treatment (enrolled) and the change in blood pressure using the data collected through the program application and found that task-sharing with private pharmacies can lead to early detection and improved control of blood pressure in a resource-constrained setting. However, additional strategies to increase patient screening and retention rates are needed to ensure sustained health benefits.

Impact evaluation of a for-profit organisation on TB care in India: A PPSA model
Tuberculosis has the highest disease burden globally. In India, the public sector delivers standardised TB care through the Revised National TB Control Programme but the private sector is the first point of contact for healthcare for 50-80% patients. The National Strategic Plan (NSP) highlights that the current scale of private sector engagement is insufficient, relative to its size. It is, therefore, crucial to engage the private sector effectively to achieve universal access to quality diagnosis and treatment. ISB MIHM as part of a study funded by CHAI estimated the impact of their two pilot programmes focused on home-based service delivery with Tata 1mg: Drug Delivery and Service & Drug Delivery. We found that while the Drug Delivery pilot resulted in better health outcomes, largely due to the efforts of the existing JEET personnel. On the other hand, despite a lower per-case cost, the impact of the Drug and Service model was not clear, possibly due to outsourcing of on-field services such as counselling, sample collection, and more.
